Most residents at Peaceful Lodge utilize state-supported programs to make their care affordable. We specialize in the administrative coordination required for these specific pathways:
For seniors 65+ who require a nursing facility level of care
but choose to live in a community setting.
For individuals under 65 with disabilities who require help with daily activities and specialized caregiving.
For those requiring specialized support and cognitive engagement following a brain injury.
We coordinate with MSHO (Minnesota Senior Health Options) and major providers like UCare, HealthPartners, and Blue Cross Blue Shield.
Contact your local county social services office (Hennepin, Ramsey, Washington, etc.) and request a “MnCHOICES” assessment. This is the mandatory first step to determine waiver eligibility.
Once the MnCHOICES assessment is complete and your waiver is approved, reach out to our admissions team or tell your Waiver Case Manager or care coordinator that you wish to explore Peaceful Lodge in Oakdale. So that a formal referral can be submitted to us.
Our clinical team will review the referral and perform a personalized assessment to ensure our Customized Caregiving plan perfectly matches your loved one’s needs.
We work directly with your Case Manager to finalize the “Service Agreement.” Once the county authorizes the funding, we set a move-in date and begin the Peaceful Lodge transition.
